Champion Dental Limited – Terms of Trade

  1. All fees are inclusive of GST unless otherwise stated (at the current rate of 15%) and valid for a period of 30 days from communication.
  2. Estimates provided in advance of treatment may vary due to unforeseen events during the treatment. Your dentist will advise you of any changes to your treatment plan, seek your consent along with full costs before proceeding to the next stage.
  3. We accept payment in the form of cash, EFTPOS, Visa, MasterCard, Q card, Direct Credit. We may have more options from time to time. Please inquire with your dentist or the receptionist at our dental surgery.
  4. If you wish to pay via a card not present transaction (over the phone), you acknowledge that you confirm & accept these terms of trade.
  5. Depending on the nature of your treatment, we reserve the right to request for a full advance payment or a part advance payment. If no advance is requested prior to the treatment, all our dental services are to be paid for immediately following the appointment.
  6. Any cancellations made on the day of your scheduled appointment or failure to attend your scheduled appointment may incur fee of $50, if we are unable to fill the appointment with a suitable alternative. Depending on the time commitment we reserve the right to charge an additional $30 per 15 minutes for appointments greater than 30min.
  7. If you have concerns about the financial commitment resulting from your planned treatment please discuss this with your clinician or our reception team prior to having treatment. They will advise you accordingly, and we will do our best to find a suitable payment solution.
  8. Champion Dental Limited offers WINZ quotes, to allow you to receive the treatment you need. If your treatment is denied by WINZ you are liable for the initial consultation fee. Please advise our team when booking your appointment that it is for a WINZ quote.
  9. We reserve the right to apply an administration fee of $25 for any unarranged balance outstanding 30 days after treatment.
  10. Any dental treatment undertaken with a delayed payment plan must adhere to terms explicitly agreed on your delayed payment agreement, or with the Champion Dental Centre team. Any failure to follow the agreement by you will lead to normal recovery procedures along with relevant administration fees and delayed payment fees, as applicable.
  11. If you have not met your payment commitment for more than 60 days, we reserve the right to use the services of a debt collection agency or other legal avenue to obtain payments. You are advised that such actions from our end might adversely affect your credit rating in future.
  12. Any costs incurred to recover outstanding payments will be fully invoiced to you.
  13. The patient authorises Champion Dental Centre to collect, retain and use personal information about the patient for the purposes of assessing the client’s creditworthiness. The patient authorises any other company or person to provide Champion Dental Centre with any information of the type referred to above that may be requested of them. The Credit Reporting Privacy Code 2004 has been issued under the Privacy Act and provides specific rights in relation to personal credit information held by credit reporting agencies as per the Privacy Code at www.privacy.org.nz
  14. Champion Dental Limited reserves the right to stop your dental treatment or not deliver outstanding treatment, where payments are outstanding. Any dental treatment stopped will not affect our right to collect outstanding payments.
  15. Should one of our staff suffer a needle stick or laceration injury from an instrument used during your treatment, you consent to a blood test. This is to allow Champion Dental Limited to provide support & care to our staff & contractors.
  16. Champion Dental Limited is bound by and will adhere to the Consumer Guarantees Act, 1993.
  17. You are not entitled to any credit or refund in the event of any dental treatment failing due to your negligence or due to clinical risks explained to you in advance, to which you have given your complete consent.
  18. If our terms of trade are to vary they will be kept at www.championdental.co.nz/terms-of-trade and also available in our practice. Proceeding with treatment amounts to agreeing with our current terms of trade.
